2. Print this form for your own records before submission. This form is only a REQUEST for RA number.
3. Once confirmed an official RA form and number will be sent to you. Allow 2 working days for processing of your RA.
4. Please contact the warranty dept. for queries relating to your submission.
5. Before shipping ensure all manufacturer's warranty terms and conditions are met.
6. Before shipment the relevant RA number provided for the product is included here.
7. For shipping ensure RA item(s) are properly packaged for anti-static protection and prevention of movement and shock.
8. Physical damage caused by your courier is your responsibility.
9. The RA number and details should be clearly displayed on the outside of the box when shipping.
10. Submission of this form confirms acceptance of the warranty terms and conditions.
